Read carefully before signing or initialing this document. If I am a minor, the consent must be signed by a Parent or Legal Guardian.

I understand and fully agree that neither SPOT DIVING, the instructor (guide), the Diving Operators associated with the underwater diving activities, nor any employee are responsible, directly or indirectly, for any injury, harm, injury, drowning, damage, or loss that may occur to me, my family, or my heirs during participation in these underwater diving activities, from the moment of land and sea transportation connections, foreseeable and unforeseeable damages, or as a result of negligence by either active or passive parties.

By this statement, I affirm that I have been informed of the inherent risks associated with underwater diving activities and participation in them. I also understand and acknowledge that diving with compressed air, enriched air (Nitrox), TRIMIX, or other gas mixtures supplied through an open, semi-closed, or closed circuit such as REBREATHERS may involve certain physical risks during dives, such as: cuts or bruises, possible attacks by marine life, entanglements in underwater debris, decompression sickness, lung over-expansion injuries, oxygen toxicity, narcosis, embolism, hypoxia, hypercapnia, or barotrauma, all of which require treatment in a hyperbaric chamber and medical facilities. I understand that underwater activities may take place in remote areas where it could take significant time to transport in case of any of the aforementioned injuries or decompression illnesses requiring treatment in a hyperbaric chamber and medical facility. Aware of this, I choose to proceed with the underwater diving activities in the absence of a hyperbaric chamber in close proximity to the dive site.

I understand that these underwater diving activities may take place at depths where it is not possible to safely perform ascents in case of emergency, run out of breathing gas, or face any other unforeseen situation during the dive. Therefore, I assume responsibility in the event that any of these situations occur, which could represent a decompression sickness or pulmonary over-expansion due to uncontrolled ascent. Likewise, I am aware and honest about my level of diving certification or the necessary experience to perform these diving activities, which SPOT DIVING and the associated Diving Operators have previously explained to me clearly and satisfactorily, including the recommendations, requirements, experience, and certification necessary to carry out these activities safely.

I agree to defend and indemnify SPOT DIVING for any legal or juridical actions taken against it by my family, heirs, or representatives that may arise directly or indirectly from my participation in the underwater diving activities, even if they are false or fraudulent.

ANY ALTERATION OR CHANGE TO THIS LEGAL DOCUMENT, OMISSIONS FROM PREVIOUS REVISIONS MADE TO IT REGARDING THE UNDERWATER DIVING ACTIVITY, WILL NOT BE ACCEPTED.
